Abstract

PT. PAL Indonesia is one of the maritime industries owned by the Indonesian government. One of the manufacturing processes in PT PAL Indonesia uses a welding process as a core process in addition to other processes. However, in the welding process, disability is often found. One method of quality improvement is six sigma which focuses on reducing the level of disability. Six Sigma has 5 phases, namely Define, Measure, Analyze, Improve, and Control. In the Analyze phase use the tools Failure Mode and Effect Analysis (FMEA) and Fault Tree Analysis (FTA). This study aims to determine the causes of disability, type of defect, DPMO level and Sigma level in the welding process. The results of this study are that the causes of welding defects are human, machine, material, method and environment. The DPMO level on ATR 2 (P) is 27350.7 per million opportunities with a Sigma value of 3.42 and the process capability value of 1.15. In BR 4 (S) the DPMO level is 23541.6 milion chance defects with Sigma value 3.48 and process capability value 1.31. The results of the FMEA method show two types of defects with the highest RPN values, namely Porosity values of RPN 256 and Bead appearce values of RPN 240. The FTA method shows both types of Porosity defects and Bead appearces having relevance, namely due to human error and poor material storage. Improvements performed based on porosity defects and bead appearances are good and correct methods of electrode storage and supervision of contract welder recruitment.
